This is going to be a silly question probably but is it possible to leave the needle in when inserting an iv catheter? I've heard it happening with old equipment but with what we have now I can't see it happening especially with safety features to stop us from getting poked.

With the devices we use, no. You have to withdraw the needle until it is fully engaged in the safety device before you can unscrew the rest and use the catheter.

I can't see how you could do that with any of the products I've use for peripheral IVs.

It's made by BD and if you didn't know to pull back on the needle (which sticks out behind the hub), a newbie could easily forget to remove the needle.
It's not meant to be left in, just to clarify. My only point is that it could be done on accident.
