We use Hendrich II Fall risk assessment on our floor.
Many pts I can justify the 4 points for impulsivity and 1 point for just being male.
Now we're at 5 points which is technically high risk.
My opinion Hendrich II is not appropriate to use on Psych floor.
Your thoughts?
Can you have someone score more than 5 and not make them Fall Precautions or are you putting yourself at legal risk if you don't.

I haven't heard of Hendrich II, so I searched online for it. All I read said it's for elderly people, and should be done at least yearly.
I'm confused. Is your psych floor geri-psych? Since I can't find an actual copy of the tool, I can't form an opinion about whether it's appropriate or not. What sorts of things does it ask/assess?
