Middle and maiden name, what is the difference?

:sofahider I'm really embarassed to ask this question but what is the difference between middle name and maiden name? In the New Mexico NCLEX application form, it is stated that I have to provide both middle name and maiden name. My mother says middle name and maiden name are the same and that I should fill up her maiden name as my middle and maiden name. But my father says they are not. He says that maiden name is the former last name of a married woman. I'm still single, so what is my maiden name?

I'm also having difficulty filling up my first name. I have two names. Someone told me I should write in my first of two names as my first name, and the second as my middle name. I've always written my two names as my first name.

It's alright to ask, don't be embarrassed. In the Philippines, your middle name is actually your mother's maiden name that's why it creates a little confusion when you try to do it how the people in the US do it. Say for example your given names are "Mary Jane Smith", in the Philippines, you will have two first names, but in the US you won't have two first names but rather, Mary will be your first name, Jane your middle. So in the US, you could actually write your name this way: Mary J. Smith. However in the Philippines, usually, when you're asked to write your legal name, chances are your name will be written this way: Mary Jane G. Smith (assuming your Mother's Surname/maiden name is Green). See the difference? The two given names are not abbreviated and the mother's maiden name is always included but abbreviated when writing your name. I hope I explained it clearly. I know that you might want to do it the American way but you might encounter problems with security checks and immigration if you have a discrepancy in your name. So, just follow exactly what's on your birth certificate. If it says that you have two first names, so be it, and when they ask how is that so, just say that you copied what's on your legal papers so as to avoid inconsistency.
