Insulin Pump Question

I have a student on an insulin pump. I have some questions about the dosing. The student's accuchecks go directly to the pump, which administers bolus. My thing is..I can't get the amount of insulin to match what is prescribed on doctor's orders. According to my calculations based on orders student should get more insulin than he is. Does anyone have experience with pumps? Maybe there is something I am not understanding? I understand the basal rate factors in, but even with that numbers don't add up.

The pumps are calibrated by the doctor. It factors in the student's blood glucose at that time, how long ago the last bolus was, basal rates, the amount of carbs eaten, etc to determine how much needs given. Most will also receive a long acting insulin in the mornings as well so that may also be taken into consideration. Plus that's always being adjusted based on student response. Maybe what you're trying to figure is a formula that's a little outdated according to the pump calibration. Our orders usually say use the pump wizard. You can always call the dr and ask. Most are happy to help. Either that or the parent.

Thanks Nurse ABC. I am waiting on a call back from the clinic. I received new orders this week so that's what I was basing my calculations on.

It is going to be very hard for you to calculate up exactly what the pump should be giving. Like Nurse ABC said, the pump takes into consideration many other factors besides the carb/insulin ratio. I have 2 kiddos on pumps....one is Medtronic and the other is Omni Pod. The parent can change ratios if they want......I'm not saying they should but it's not complicated to change the pump settings. I have learned to trust the pump unless something seems WAY OFF. Good luck! :-)
