inquiries on application for nclex exam in NM

Hi guyz im a foreign nursing grad from philippines. I am planning to take nursing licensure in new mexico. i already have the application form but i got a few questions and its under the examination payment form which is,

they are asking for my SS# but i dont have one. is it ok to leave it blank instead?

also for the examination fees, i know my foreign transcript needs to be evaluated but theres this option in which i am confused of which applies to my situation and these are: (i am a foreign grad who is just applying for licensure by examination)

__RN $110.00

__LPN $110.00

__Foreign transcript eval $ 50.00(please include in the same payment with application fee

__Foreign Transcript eval only (if not applying for exam)

lastly, is it ok to apply for NM licensure exam without local board here in Phils coz in their instructions they are just asking for the transcript of records sent by school registrar to BON of NM.
