Our PACU also does Pre-op Check-in including Inpatient. We are looking for ways to revamp our Pre-op/Pre-Procedure Checklist to make the process more user friendly for the RNs on the units that are supposed to be completing this. I was wondering if any of you have a good system and/or a good Pre-op/Pre-Procedure Checklist I could take a look at?

Is your pre-op checklist in the computer or on paper, we have had a lot of discussion about this at my work and have ended up with both. The RN on the floor completes the computer checklist including labs, vitals, jewelry off etc. The PACU nurse completes the paper checklist which is H &P, Consent, ID band, Allergy band, warming gown, site marked, anesthesia eval etc. etc.

What I've found works best is an all-in-one document that covers the entire perioperative duration. Page one is admission, patient questionnaire and history, securing of valuables and clothing etc and last oral intake. Page two is consent. Page three is anaesthesiologist's pre-op visit, orders/premed and pre-op check list, which is completed by the admitting nurse, reviewed by the receiving nurse in the pre-op area, and again by the scrub nurse prior to transfer to the OR. Page four is the operation form, five is PACU, six is post-op ward notes and instructions for discharge.
