I included mine. Guys what is this details of identity card or identity number issued by your government? Is this my passport?

i ask the VIA call center about this and the agent said, an identity card can be any government issued ID, like PRC license.

I left mine blank but.my application is still for pick up tomorrow. How about drivers licensecan that be an identity card also?

to chelseaxyrene, yes that is what the call center agent of VIA said, any government issued ID. that includes driver's license. you can call the call center if you want.

hi! i would just like to ask some questions about the medicals

- did you print the forms 25 and 150 before going to the center?

- what else did you bring along with you for the medicals?

- do we still need an appointment before going for it?

- where did you do your medicals?

Why do you need a cenomar by the way

I'm wondering also. i talked to my friend and she said if you put single you will have to send cenomar. If you put.married you will have to send marriage certificate. She sent one herself. So i don't need this? Thanks!
