Imaging for recurrent UTI in male?

Specialties NP

I have a male in his 50s, negative for STI, who keeps getting recurrent UTIs. I'm referring to urology, but I'm not sure whether there are diagnostics I should do prior to the referral. Any suggestions?

Thanks

ER is different. When I worked ER, USD was not always available, CT always was. For me, that was the deciding factor!

In an office setting, where its not an emergency setting, you can schedule USD in next 48-72 hours.

I'm in nephrology - get a BMP, UA, Urine Culture

Renal USD is fine. No need to image further and no need to expose them to CT.

Send to Urology.

I agree. Let urology run the gamut, after that they will probably put him or trimethoprim or cipro and send him back.

I think you covered all the possible imaging, but just wanted to mention always have bladder CA in your differential. My father was treated for chronic prostatitis for 3.5 months by his primary and urology as well before they decided to image him. Found several tumors. His only symptoms were UTI type symptoms, he did not have gross hematuria. I wish we had found it sooner. He's sustained a lot of damage to his bladder. :(

And a thickened bladder wall would most likely be picked up on renal USD. However, if nothing is picked up on renal USD, your prompt referral to urology would.

I'm so sorry for your father.

Thank you TraumaRUs, I appreciate your kind words...
