Each LVN-to-RN bridge program will have its own unique prerequisite classes and other requirements that you must meet before you'll be eligible to apply, even though you'll already be an LVN.
For example, one bridge program in my area states that the LVNs must complete A&P I, A&P II, Chemistry, and Algebra before being allowed to apply to their program. Another bridge program requires the A&P I, A&P II, Nutrition, Microbiology, Psychology, and Growth and Development before applying to their program.
Some bridge programs will require you to take the NET test and achieve a high score on it. Other bridge programs will require you to take the HESI test and achieve a competitive score.
So, here's the answer to your question: you must meet the requirements of each specific bridge program before you can apply. An LVN-to-RN bridge program will not simply accept you because you are an LVN; there are prerequisites and test scores you must attain first.

Not necessarily. You have to apply to the program, following their guidelines. If more people who are more qualified than you apply, chances are you may not get in or may have to wait, just like with a regular program. There are many of these programs that have more applicants than they can accommodate. You need to explore your options with an admissions counselor from the programs that you are interested in. Good luck.
