What is your role in the 504/IEP process? I have been sent forms in google docs from the person in our school that initiates evaluations to begin the process of requesting 504/IEP's, to fill out the medical information. IE vision, hearing screenings, medications, and medical diagnoses. I am happy to fill in this information if I have it. Often times I don't have it. I can request it but I do not feel comfortable filling in diagnosis or meds unless I have been given this information from their doctors office. As we know parents and kids aren't always accurate in what they report to us. What would you do? Or are you involved in filling in any of this information?
What forms do you use and how do you get your forms to students to fill out medical information? I think our district uses blue cards but each nurse does things totally different at each school. I am going to look at our new student orientation packets but we have many students that I just don't have information on. they come in throughout the year and I may figure out by chance that they have an inhaler or they have a food allergy but parents aren't giving me this info in the beginning. So then I have to call and request they get an order.
Thanks for your help. the staff in my building make me feel like I should know every detail about these kids home medications that they don't take at school but how would I know if they aren't voluntarily giving me this information. I would like to know and have it in the computer readily available for when they come in so I have the whole picture on them, but that just isn't the reality. Not to mention it can change and they don't update me if it doesn't affect their day at school.

I only manage and write what I call the strictly medical 504s - basically students with 504s for diabetes, seizures, migraines, etc.
I do not manage 504s for students with ADHD or hearing loss - but I am part of the team. I work our SPED director, principal, and the student support liaison that is assigned to the student.
For IEPs, I only participate if my role is part of the IEP. I will pass along health info and do additional screenings if needed.
