IEP health review

These are the forms that I use. I also attach a consent so that I can talk to the doctor's office as well.

Awesome! I call most parents but I'm thinking I may want to start sending home a form to fill out for annual reviews. I checked my spreadsheet and this year I have done 215 annual reviews so far (couple weeks left) in addition to 200+ initials. If I can eliminate some of the phone tag, that would be great!

I only send them home, have them fill it out, and return it. I find that I have received the same information sending the form home vs interviewing...actually, I probably get more because the parent can take their time to think about the answers. Plus, I have found that it is a much more effective use of my time. Obviously, I'll still call to clarify any questions and I'll call if I haven't received the form back, but for the most part, sending it home has worked very well for me. I think I'll be at 50 reviews by the time the year is over.

Oh my gosh....I read that wrong the first time...so you've been involved in 415 IEPs?? I know you're in a big district, but holy cow!!

It is a big district and this is only the preschool! We are 65% IEP students. I don't have to sit in on all the meetings but I need a report for all of them ?
