IDFPR ssn affidavit form

Hi everyone, 

 

I hope someone can help me out here. Currently attempting to endorse my RN license from Canada to Illinois, I don’t have an SSN so I filled out the SSN affidavit form provided by the IDFPR. I have sent it to an email an agent provided me with but it’s been 4 weeks and I haven’t heard anything yet. Is there any way I can create an account with IDFPR without a SSN or another way I can submit my SSN affidavit form? are they only accepting online applications?

I would say online is better cause I sent them mail and ne reply it is talking for ever.

how long has it been since you mailed it to them?

Any update on this? Try looking up your name on the license look up here if it tells you what requirements you're missing.
