Does anyone have an I.V. flow sheet that works well for them in the long-term care setting? I am a nurse consultant for a closed door pharmacy and I have some facilities that are looking for a new I.V. flow sheet. Not much out there for ideas.
Thanks.

How about keeping it simple...just record it on the MAR and initial it q shift. We also have a IV sheet for site check, tubing change, and dressing change.

I have a good sheet. it is for site assessment. If you would like a faxed copy, you can e-msil me @ [email protected]. I am a DON and have seen and utilized many different forms. This one seems to satisfy the needs for surveys and is the least time consuming.
