I.V. flow sheets

Does anyone have an I.V. flow sheet that works well for them in the long-term care setting? I am a nurse consultant for a closed door pharmacy and I have some facilities that are looking for a new I.V. flow sheet. Not much out there for ideas.

seems pretty basic a columned sheet with one side to list the fluid and across the page columns for time started then initials the time stopped and initials again

How about keeping it simple...just record it on the MAR and initial it q shift. We also have a IV sheet for site check, tubing change, and dressing change.

yeah ours is as I mentioned with about a quarter page at the bottom

with colunm for IV start/ d/c thenthe other side for tubing drsg change date/time etc.

I have a good sheet. it is for site assessment. If you would like a faxed copy, you can e-msil me @ [email protected]. I am a DON and have seen and utilized many different forms. This one seems to satisfy the needs for surveys and is the least time consuming.:p
