I passed my NCLEX PN and lost the Papers. Help!!

I took my Nclex Pn on 7/6/2005 and passed :balloons: :balloons: But I am moving to another place and misplaced the papers I need to fill out in order to get my plastic license. Does anyone know if I can download the forms from a web site, so I can send them with the $100 dollars they ask for to get the actual license. Please help :uhoh21: :uhoh21:

You need to contact the Board of Nursing where you are trying to get licensed. I think that you are speaking of CA by what you are describing, just go to their website.

The only thing that I would suggest is to contact Peterson(?) Vue-the company that you test through-but you also should be able to contact your current state for a copy-they had to of been notified when you passed
