I understand that outcomes in a careplan are supppose to be measurable. For example: The patient will increase fluid intake to 2000 ml per day. The patient will have a respiratory rate of 12-20 per minute with no signs accessory muscle use after prior to discharge.
Recently, we've been introduced to the NIC and NOC, which are far more standardized than what I used to do. I ignore the NOC and NIC in careplan books, because we were never taught how to use it, but from my understanding, it is similar to what I used to do in my other careplans. I highly depend on my careplan books to help with my nursing careplan assignments, and anything I can't seem to figure out, I'll ask it here.
I am having trouble figuring out how to make these NOC indicators measurable. I want to make the each indicator measurable. The indicator that I have are in bold, and from what I read, they are suppose to be measurable. I am looking at it, and it doesnt look measurable. How would I reword it to make it measurable, any advice, any help is appreciated.

Safe Home Environment -
Physical arrangements to minimize environmental factors that might cause physical harm or injury in the home.
Safe Home Environment as evidenced by the following indicators:
Indicator:
-Maintains a safe home environment.
-Identifies available resources.
-Uses available resources.

Pt will maintain a safe home environment by removing throw rugs by next visit 09/21/05.
Pt will state 3 available resources to nursing student at the end of todays conversation 09/17/05
Pt enrolls in stated resources with the assistance of the nursing student on 9/17/05
This has always been a difficult thing for me too. But I think those are specific, measurable, accurate, realistic and timely = SMART
