Anyone have good generic care plan for Aspirin/Plavix? I have a pretty specific care plan for Lovenox and Coumadin, but I'm looking for something a little less involved for our residents on daily dose of aspirin. Anyone?

I haven't thought out a care plan since nursing school. I was thinking in the line of "At risk for impaired skin integrity"(bruising easier). Is there an "At risk for bleeding?" I can't remember!

A care plan based on a drug? Do you mean a nursing diagnosis for your pt's care plan based on the fact that the pt is using ASA and/or Plavix?

Lets see ... how about:
1) Risk for Injury, Poisoning R/T Altered Thought Process
2) Risk for Altered Tissue Perfusion
3) Risk For Impaired Skin Integrity
??? Thats about all I can think of directly linked with ASA therapy
