So I had an interview with the nurse manager for a position in the ED. She said I would hear a response within a week from the Talent Recruitment Center. I hadn't heard anything so finally called TRC and they told me I have a phone interview with someone from HR next week. It will last about 20 mins and will be basic background information. They just "need it on file" and will forward it to the nurse manager and she will inform them of where to go from there. I'm confused as to what this means?! Does this mean I basically have the job or does everyone who has the face to face interview also have the HR interview after? I also don't know how I should prepare for this/what kind of questions they will ask.

If you have to ask, then there is doubt. Never consider that you have a definite job offer until you get something in writing.
But it does sound promising that you ARE going on further in the application process.
However, do not stop and burn any bridges by holding your breath for this ONE chance. Continue any other job search opportunities if they occur.
Can't help you much beyond except to say have your resume in front of yourself. Jot down some notes. Be strong about your clinical experiences and how you can be an asset to them. Breathe, stay calm (never let them see you sweat!), put your best effort out there, and GOOD LUCK.
PS - I find the term TALENT Recruitment Center funny. Like you should be doing a tap dance or some karaoke ! !
