I recently applied for a job with a hospice company for the Continuous Care Coordinator after 3+ years as a continuous care LPN and recently getting certified in hospice and palliative care. I was wondering if there was any nurses out there that is familiar with that title and what does the job truly consist of, does it go by another name and what is the salary like?

Are they looking into a LPN? I am not sure but I guess it is probably a mix of coordination and bedside nursing care depending on the size of the hospice agency. It could involve the scheduling but I think it will be a lot of bedside nursing. Don't they have a position description attached ??

Most large hospices have a continuous care staff. There is quite a lot of administrative work in coordinating who goes where each day. There is also a lot of documentation that must be done by the continuous care staff and someone needs to coordinate all of that. Most continuous care nurses are LPNs because the don't have to be RNs, and the hospices would rather hire cheaper labor to do the work. The coordinator is also frequently an LPN. It may or may not require direct clinical care, but mostly you will be in a cubicle coordinating staffing.
