I am a symptom management LPN for home hospice. Are any other nurses doing this kind of work?

I work for a small hospice agency, very rarely do we have situations when continuous care is required for symptom management.
I've had several cases where the visits are multiple hours long to address symptoms but only one in the past year that truly met the requirements for continuous care.
I'm curious, what types of patients you are seeing most often requiring continuous care?
Initially I was assisting families for one night that were up for 2 days or more managing symptoms. I would write out a plan of meds to give and teach how to care for pt. But now, its 911 calls per family request when I get there to do continuous care for a patient that is not a DNR.
