Home Health "Cheat Sheets": Any Suggestions?

So I'm a newbee to the home health world and have been with the agency I'm at for less than a month. Earlier this week I had OASIS training and learned all about it and realized all the information it includes. I was wondering if any of you have some type of a cheat sheet that you use when you do a SOC visit to help keep all of your information in one place for when you go back to document later. The nurse I'm training with gave me a sheet she uses to keep frequently needed patient info in one place but said she doesn't really have anything she uses for OASIS since she's been a home health nurse for so long she knows the questions fairly well and knows what to ask on that initial visit. Any information/advice would be very helpful!

Hi I also just joined home health! I am looking for a similar document, and I have been going through the threads. I found talk about someone's "cheat sheet" but only requests for email, and its old. If anyone currently has such a paper to share, we would really appreciate it ! I am excited to be in home health and may just sit down with oasis and write a "script" of what questions I see myself asking to get the info, and take it from there- not that I'll read it word-for-word, but the questions will be there if I get stuck and to guide me in the beginning. But would rather not reinvent the wheel, if you know what I mean!

We are just transitioning to electronic charting and I've been thinking about developing a cheat sheet. It would have to follow and include the questions as they are listed in our software. I already have our old paper oasis but it's too out of order to be helpful, I don't think a one size fits all cheat sheet is possible. I'm just going to write out the questions as they come (at least for the ones I need an actual figure) as well as a patient questionnaire for the informstion they can easily provide without explanation.

Reinventing the wheel, at least one like this that isn't not complicated, can help cement the assessment for you.

This is what I used see attached below, Only valid for the SOC (Start of Care) info. Then transfer to the Oasis.
