I am starting a HH nursing job next week and I was needing some advice on rather to take pay for mileage to to take more per visit and then deduct mileage on my taxes...just a little lost. Some help please

You might want to seek the advice of your tax advisor on this, or take a look at the subject in Publication 17, free from the IRS.
