I personally try to handle all situations without involving mgmt, if possible. The incontinent patient seems like the result of a lack of delegation. Sure it is the nurse's responsibility to make sure it is done, but there is a guilty CNA out there whose job it was to handle that, especially when the nurse cant even handle her own responsibilities. Is this nurse lazy, or is her assignment too large?

I have no problem with confrontations. I would talk to the nurse and the CNA. If the nurse is too slow to handle her job, then i would tell the CNA she needs to step up and do her job without having to be told to do so.:)

Sounds like your manager is a pansy. They should be interceding as this is extremely incompetent patient care. It is not your job to deal with incompetence. It is theirs. Period.

Of course I tend to be a bit outspoken so I'd have called her on the carpet immediately - by phoning her at home. "You left Mrs Jones in soiled linen and absolutely no oral care - was there any reason for this?"

Confrontation sucks, but it is your job as a nurse to advocate for your patients and that includes confrontation sometimes. With coworkers, doctors, family, whatever.
