i am an lvn and am currently transitioning to rn. i can't remember how to reconstitute meds... can anyone out there help?!?!?!

first of all, let me welcome you to allnurses! i am currently an lpn student so this is still fresh in my mind. how i learned it, you look at the back of the powered vial and it should tell you how many cc's of liquid and how much of a certian kind of liquid (sterlie saline, bacteriostatic water, etc) to make the ordered dose. then you draw up the liquid into a filter needle, put it into the vial of powered medicine and then draw up the reconstituted med into the filter needle, then change the needle and your ready to go! hope this helps! :)

Each vial you reconstitute has package instructions on how much diluent to use, as well as what to use: NS, sterile H20, etc....

how are doing on care plans. someone told me that they had note cards to help with a care plan. do know or anyone out there.. thanks

Thank you!!! I knew it was a pretty simple procedure... I just couldn't for the life of me, bring it into focus. You must remember hoe it is to get SO incredibly bogged down with information that the "little" things slip your mind :)
