HELP! having trouble

I am a nursing student given her first case study! Our teacher told us she doesn't expect much since we do not have the knowledge base yet, but I am researching it anyway. A 16 year old delivers baby at home in bedroom. Parents find her and bring her in ER. Baby is premature and in resp. distress and mother has uncontrolled breathing, Mother blames boyfriend who denies any responsibility to the baby.

I have to describe a phase of interview process, nursig actions, factors that would influence type and amount of data collected and develop a plan of care.

I have an idea as far as the bleeding, the baby is resp distress and calming down possibly separating the mother if all else fails but I'm not sure. Thank you!!!

Mom with uncontrolled bleeding or breathing?

Get a few consults going... a neonatologist needs to see the baby ASAP, for meds and maybe setting up a vent. You'll want the ERs OB cart or whatever in the room, there's probably a warmer kept nearby and it may have all the newborn resucitation equipment needed. Biggie is warming right along with the baby's ABCs.

Of course there will be a social services consult... I wouldn't let security wander too far away, either, some families can get touchy!

Are you presumed to be the ER nurse or OB nurse? The OB nurse will be getting a thourogh history and creating a longer term care plan and teaching plan. Think of all the questions you'd have if you were 16 and just had a baby! The nurse will be planning to address them all... a 16 y/o won't think to ask! The ER nurse won't be getting too much info before the couplet is transferred.

I wonder how early this kid is, any interventions at home before he started breathing, any prenatal care (well, I can guess)

Did mom deliver the placenta, etc yet? cord cut? lady partsl tear?

I believe along with physiological and safety needs you also need to determine the physchosocial needs of this patient. She is 16 years old and had her baby at home in her bedroom. How does she feel about being a mother, is she afraid, does she feel abandoned by the father and also where is she at as far as developmental theories go. All this together will help you to plan her care. Will she have a diagnosis of Ineffective coping related to ?. Just a thought hope I am not way off base here. I am also a nursing student and just wanted to bounce this around in the discussion:)

I’m in pre-nursing classes, and ...these type of lessons - case studies seem like they will be fun, interesting, and hard work.
