Help with reinstatment letter.

Does anyone know how to write a reinstatement letter to a nursing school. I withdraw from one of my classes due to illness and now I am at risk . I may be kicked out of the program because I withdrew. I do have supporting documentation from my doctor. How do I go about writing this letter.

I am having a difficult time finding any examples for reinstatement letters, however I read a few email exchanges. Would it be possiable to get a copy of it?
