So it's been a while since I've visited this site. As my first year of nursing school is coming to an end, I can happily report that I have survived! There are only 2 finals left until I have officially completed my first year. Sufficient to say, I am absolutely ecstatic and have shed more laughter and tears over this past year than in my lifetime. I am thankful for the quirky friends I've made and the awesome staff that my school provides, as well as the unforgettable patients that I have encountered during my clinicals. I am now looking for a job for the duration of the summer, and hopefully throughout the school year (depending on scheduling flexibility), and am doing some touch-up on my resume and references. I have my resume all set, but it is my references' page that is giving me trouble. I would love any feedback that you could offer! Here is what I have so far.
Professor xxx
Clinical Instructor/Professor
School name
Address
Phone number
Professor xxx was my clinical instructor for OB/Peds and professor for Pharmacology and can attest to my character and my diligence in the academic setting.
Clinical Instructor
School Name

Professor xxx was my clinical instructor for Med-Surg I and II and can attest to my character and my skills in the clinical setting.
Ms. xxx
Restaurant Owner/Manager
xxx was my manager at "name of restaurant" and can attest to my work ethic and character.
I changed up a few of these details, but I was wondering if the format was okay. Additionally, I am lacking in eloquent speech, so I put the gist of how I know each of these references, so if you could offer any suggestions as to how to fix up these references, I would appreciate the feedback.

You can do it like that, which is fine. My reference page looks like this:
Scarlett O'Hara RN, BSN, MSN
Professor at Tara University
