Help with professional reference list! Please?

I have a job interview tomorrow and I am putting my reference list together. How do should I write them? I kind of have a template but I'm not sure if this is right. Thanks in advance

That is good enough. Just have this down on a piece of paper in case you are asked for this info. Make certain you ask, if references are not brought up. Some employers will tell you that they don't want a list from you, they call former employers and talk to the supervisors.
