please help with the nursing care plan assessment
case study:
Thi Nguyen is a 81 yr old widower-diagnosed with Parkinson disease 4 yrs ago. Also diagnosed with hypothyroidism 4 yrs ago-now controlled on medication. Moved from Vietnam in 1946, reads and write English well and maintained strong Vietnamese accent.4
Lives alone in 2 storey home-lived with his wife until she died 12 yrs ago-since then independent-no children-his brother Vinh and family living nearby.
Don't want to bother them by involving-small income from his superannuation-careful with his money- does not want to receive financial welfare from government-it allows him measure of financial independence- because of his reduced investment, now only able to cover his expenses- usually goes on holiday once a year but unable this year due to altered mobility.
Has slight tremor in hands and getting worse- reports to difficulty in doing simple tasks such as cooking, putting on buttons and tying shoe laces- wonders how he can face the uncertainity of his future due to worsening process
Current medication- thyroxine: 100 mcg daily- carbidopa/Levodopa: 25/100 q8h, Entacapone: 200 mg q8h
GP asked community nurse where you work to visit Thi to assess whether he has problem managing his medications.
have to write a care plan using millers functional consequences of ageing as theoretical framework and levett-jones clinical reasoning mode to guide nursing care plan, intervention and evaluation of care.
due in next 8 hours/havent yet started/ have no idea how to start the essay

The first thing I hoped you've learned is that waiting until only a few hours before an assignment is due to start it is a bad idea. These habits will not be helpful throughout nursing school.
Beyond that, use the search bar in the upper right of the webpage to search for nursing care plan and GrnTea and Esme- these two members have posted a lot of very helpful information on care planning/nursing diagnosis.
