Hello I am currently doing a case study for one of my classes on renal failure and I was wondering if anyone had any opinions on my nursing diagnosis??
Here they are
1. Excess Fluid Volume related to excessive fluid intake as manifested by peripheral edema, shortness of breath, and statements from client stating that he has difficulty staying on his 1L a day fluid restriction.
2. Fatigue related to altered metabolic state as manifested by decreased energy,and anuria.
3. Knowledge Deficit related to the process of dialysis and medications as manifested by statements of clients stating "I'm fearful it will clog up. Its my lifeline".
4. Imbalanced Nutrition: Less than body requirements related to anorexia, and nausea as manifested by loss of appetite and loss of 9kg over the past 8 months.

1. Excess fluid vol r/t fluid intake more than output aeb peripheral edema, crackles in lungs, sob, and client expressing difficulty adhering to 1000ml FR.
2. Fatigue does not cause anuria, RF causes anuria. I would say Fatigue r/t disease process and dialysis aeb excessive sleeping, weakness, interference with adl's.
3. Fear rather than knowledge deficit. It is a real fear.
4. Is fine.
