Help needed with resume-same job/ parent company, different state

I am hoping someone can help.

I am trying to update my resume. I started a job working in a hospital that is part of the same parent company as my previous hospital job in a different state. I am doing the same job, just a different facility, b/c I moved. There was a lapse of time in between the two jobs though. I am wondering how I should indicate this on my resume. Is there a way to link the two facilities in one or do I list them separately and try to come up with bullets that are different. ( quite challenging since it is essentially the same job). Additionally, I started subbing as a school nurse when I started working at my current hospital. I am not sure where to list this either....
