One of the things that has reduced medication errors is a computerized
medication program. You have to scan the patients arm band which
through bar code(which is his social security number) tells you who
he is identified as. It then brings up his medication record at the
window of time which you have set. If nothing is to be given
it will tell you.

We use the computer system where all meds are scanned and the pt is scanned. I also always call my pt by name and go through the meds I am handing them (name, dose, what it is for) and if the pt questions anything (color, dose, "I didn't take that at home") I double check the MD order (Cardiologist are know for adding meds for BP, cholesterol, etc and the pt finds out the first time the nurse comes in to give it).
