Hello,

I'm working on a care plan for school and can't come up with a long-term goal for risk for falls. P/t is 57 y/o post-op for benign suprasellar meningioma. P/t has Hx of intermittent seizures, generalized weakness, unsteady gait, and slurred speech. I really appreciate the help! ? Thank you ^^

You have done the first part by identifying the problem! What's your next step?

Writing a Nursing Care Plan

Step 1: Data Collection or Assessment

Step 2: Data Analysis and Organization

Step 3: Formulating Your Nursing Diagnoses

Step 4: Setting Priorities

Step 5: Establishing Client Goals and Desired Outcomes

Short Term and Long Term Goals

Components of Goals and Desired Outcomes

Step 6: Selecting Nursing Interventions

Types of Nursing Interventions

Step 7: Providing Rationale

Step 8: Evaluation

Step 9: Putting it on Paper

When I was in nursing school a million years ago I had a book that was called The Nurses Pocket Guide to Care Plans, Interventions and Rationales, By Mary Doenges: I carried it around like a Bible and it  still has a place on my book shelf
