We use two different Heparin Protocols in the ER. One is for Cardiac/Neuro reasons (MI or Stroke) and we give a bolus (say 60units per Kg but a max of 5,000units) and then a maintenance (say 12units/Kg max 1000 units per hour). you need to know the PTT and the platelet count prior to giving it. then in 6 hours you check the PT/PTT/INR and adjust it accordingly.
Another Heparin protocol is for DVT or PE. there you give slightly more. but again you check in 6 hours. each hospital should have a protocol.
yes, usually its 25,000 units in 250cc for the drip and that makes it easier to know how many cc's per hour.

Does anyone know how to do a Heparin Protocol? What do I start out with and how do I plug the PTT numbers in? What is the significance of 25000u/250ml and 18u/kg/hour. If anyone knows what I am talking about please reply.
