Help with a form

I'm a case manager for a relatively new Pediatric Home Health company - they're a recent conversion from geriatric home health. The skilled nursing forms that we have for the initial visit are ridiculously long and don't apply to pediatrics. I'm trying to see if anyone possibly has a blank template for a pediatric skilled nursing visit or initial skilled visit assessment (not the full blown regular admission for PDN) that they wouldn't mind faxing or emailing. The adult forms wouldn't be applicable because they're probably full of OASIS questions.
