Someone.. I need help..

I need a guide on how i would make a phatophysiology for our case pres

i only have two days..so i would highly appreciate any help

our patient Diagnosis is "Azotemia secondary to CKD secondary to DM nephrophaty vs. Gouty nephrophaty DM/insulin requiring with neurophaty nephropates UTI hyperkalemia, Anemia"

which i believe falls under DM type 2 phatophysiology

I hope you know this makes no sense to me at all.
