Help with care plan

Not all nursing is revolved around acute care and acute care diagnoses; this is an example of primary prevention. Think of this as a well child visit, so the assumption is the child is well with no known illnesses. "Using the nursing process, describe how you would approach the pre-K child for a physical exam and prepare them for an immunization."

Assess: How would approach a pre-k child to assess them? Would you just start off with the exam? Think about the stage of development the child is in, how will the child react to strangers, strange situation? Should you introduce yourself and the environment? Where and how should you begin? Why?

Diagnose: Your diagnosis is given-need for yearly vaccine.

Plan: Keep going with the scenario. Now use one of the theorist to plug in your answers. You will learn that nursing is not a fill in blank kind of study, it is very much applying what you know in different ways. Good Luck!

ok. i am starting to understand this a little bit more. I get the part of the assessment and that child is nervous and what I should do to make the child less nervous and making the whole situation more "fun". But when it comes to planning. What exactly would I be writing about? Would I write about how i plan to give that child vaccine and make him less nervous or is it more like goals; for example "the child will show no signs of anxiety", "child will receive needed vaccine" etc. I can't believe I am struggling so much with this assignment. Thanks for all help
