HELP with my care plan!!!!!!!

Hi everyone!

I am in desperate need of any TYPE of help with my Nursing Care Plan so I would definitely APPRECIATE any type of information or tips on my care plan.

My patient is an 88 year old male has a medical diagnosis of Gout.

1) I need help on developing a 3 types of Nursing Diagnosis and etiology (related to) based on the level of priority and 1 safety priority nursing diagnosis.

***Currently, I have:

#1. Impaired physical Mobility related to musculoskeletal impairment (Priority #1)

#2. Ineffective health maintenance related to deficient knowledge regarding self-judgement (Priority #2)

#3. Chronic pain related to inflammation ....(Priority #3)

#4. Risk for Injury related to weakness (Priority-Safety)

2)PLEASE let me know what I could do to in order to improve on my care plan.

3) I also need any suggestions "Diagnostic statements" regarding my nursing diagnosis (see question #1) and must include a RELATED TO (R/T) and followed by AS EVIDENCE BY (AEB) to support my statement.

4) GOALS & INTERVENTIONS: any suggestions for 1-patient goal (using the SMART) followed by 4 types of interventions that advance my goal.

ANY SHARED HELP, INFORMATION, ADVICE, OR TIPS WOULD BE GREATLY APPRECIATED!!!!:yawn::yawn::yawn::yawn::yawn:

Are you basing your care plan on your patient's medical diagnosis, or on your assessment? (hint, it should be on your assessment)

If you are basing your care plan on your assessment, we have no way to help because you've not told us anything about your assessment findings.

I would suggest pain is priority one...next is impaired physical mobility and next ineffective health maintenance.

#1. Impaired physical Mobility related to musculoskeletal impairment or you can have pain or discomfort as evidence by grimace upon exertion

#2. Ineffective health maintenance related to deficient knowledge regarding self-judgement.....I'm not sure what do you mean about self-judgement...but I would say regarding food restrictions e.g. food containing uric acid AEB increase uric acid present in the blood

#3. Chronic pain related to inflammation AEB increase WBC...or you could say chronic pain related to increase uric acid deposits AEB laboratory result of uric acid:____ in the blood (mention the normal range) (mention the rate of pain too...)

#4. Risk for Injury related to weakness secondary to old age..i doubt gout patients are having weakness except your patient may really experiencing weakness due to aging....this one doesn't have AEB because problem is not present yet

I would do thorough assessment on patient not just knowing the medical diagnosis...because not all the signs and symptoms are visible....you can also find things not related to the medical diagnosis...I would suggest having NCP guide book too...like gordon's where i find it very knowledgeable and easy to use...there is a lot of list of interventions and goals in there....

It's hard to have the interventions and goal because I dont know the full data of your patient...

I would suggest that you you do # 3 and #4 then post what you have. No one minds helping but we should not be doing the work for you. A good med surg book will point you in the right direction.
