cannot help you unless you supply more specific information about the patient. a care plan is about determining the patient's nursing problems. that is done by using the nursing process which is our tool for problem solving. begin by doing a thorough assessment of your patient. assessment consists of doing:

  • a health history (review of systems)
  • performing a physical exam
  • assessing their adls (at minimum: bathing, dressing, mobility, eating, toileting, and grooming)
  • reviewing the pathophysiology, signs and symptoms and complications of their medical condition - you can find information about cephalopelvic disproportion by looking on the weblinks posted here:

[*]reviewing the signs, symptoms and side effects of the medications/treatments that have been ordered they are taking

from all that information that your collect you are interested in what is abnormal. the abnormal data becomes the signs and symptoms that will form the evidence that will support and prove the nursing diagnoses (nursing problems) that become your care plan.

care plan construction is presented on this sticky thread: https://allnurses.com/general-nursing-student/help-care-plans-286986.html - help with care plans
