We implemented one as well. Systems on the front, updated each day in pencil. The back has columns for procedures and their dates as well as a bit of space to sum up anything noteworthy that may be lost in the cracks over the course of the patients stay. The back portion helps alot when transferring a patient from the unit that has been there forever. I like them, I just wish folks wrote a bit more legibly. Ivanna

Does anyone use an official "Handoff Tool" geared towards critical care in their units? Yes, it's for Joint Commission...ug.
