Per the AWHONN definition, para only counts the number of pregnancies. So, when you use the full GTPAL system, it is G1 T1 P0 A0 L2. If it were a 28-week quad delivery and all 4 babies are still alive, it would be G1 T0 P1 A0 L4. Anything under 20 weeks counts as an abortion (spontaneous or elective), and from 20-37 weeks, it will fall under 'preterm', regardless of how many babies OR the outcome of the pregnancy.
Where it gets confusing is that some folks only do G/P rather than GTPAL for the sake of brevity.
