giving report??

Everyone, this is my 1st msg so bare with me. I am currently in career mobility and will graduate in May 08. I have been working in LTC for the past 3yrs and have recently transferred to the hospital GI/GU floor. I absoultly love it. I have learned more in 1 month then 3 yrs in LTC. Anyway, I am having a hard time making sure I give a good report at shift change. Some want everything even shoe size (lol) others only want the basics. I have searched this website and found some report sheets, but still not sure what to use.

I have 5-6 at a time and dont want 5-6 sheets of paper plus MARS to carry around all night. Does anyone have a multiple pt report sheet?? or info on where to find one??

I have a good sheet from work that I'll scan tomorrow as I don't have a blank one now with me.... but off the top of my head you would essentially give:

Name/room number/age/gender/code status/allergies/diet any restrictions

why are they here? and pert. pmh

then just a head to toe....ie. a&ox3, cardiac any issues, lung sounds O2 sat ox, GI/GU last bm and output, any skin issues/dressings, any tubes/drains if so what's going in / what's coming out

then any labs/tests pending or done, any change since last shift / any new orders and any prn's given

I'm sure there's more but that's off the top of my head.

Now I work with a sm group and we are a satellite unit on a Long Term Acute Care so the pts sometimes are there for a while so sometimes if I'm familiar with the pt and I had them the previous day then I just ask for updates otherwise if they're new to me or I haven't had them I'll ask for the info....
