Hello all-

Does anyone know of a book/website that helps students sharpen their S.O.A.P. charting skills?

I also noticed a website called care plans. However, it charges $20.00/yr to view their care plans. Has anyone subcribed to this site, and if so, is it worth it?

It also has other areas, but the care plans are thorough. very easy, and best of all--

FREE!!! I used them for my clinical care plans and got a 100% on each one!!!

Hi there! I just used my text book for the S.O.A.P notes. Are you having problems understanding WHAT to chart or what exactly are you looking for? I have a few suggestions for careplan websites:

This is my 2nd semester. We go to the VA (our clinical site) the end of this month and I want to be ready!!

I understand the Subj and Obj. under the assessed data.

At times, I have a hard time choosing a (the best) Nrg Diag.

The expected outcomes are so-so. I need to keep in mind (TOMM'S Real Patient)

Nrg Interventions, and Rational I understand.

The "A" portion of the eval throws me a bit.

It would be nice if I could chart scenerios on my own and then have some way to check to see if I am on the right track. Esp w/ the SOAP/Eval portion.

I have Ackley & Ladwig's care plan book but I am not finding where the SOAP/Eval part is listed. :rolleyes:

Here's an example of SOAP that I got out of my Fundamentals of Nursing book:

S - "I'm worried about what it will be like aftre surgery."

O - "Client asking frequent questions about surgery. Has had no previous experience with surgery. Wife present, acts as a support person.

A - Knowledge deficit regarding surgery related to inexperience. Client also expressing anxiety.

P - Explain routine preoperative preparation. Demonstrate and explain rationale for TCDB exercises. Provide explanation and teaching booklet on postoperative nursing care.

This is from the Potter Perry book page 507. Hope this helps!

S - Subjective data (verbalizations of the client)

O - Objective data (that which is measured and observed)

A - Assessment (diagnosis based on the data)

P - Plan (what the caregiver plans to do).
