a "pack per year" history on smoking?
I have a careplan to do and need to insert that my pt is a "recent ex smoker with a ?? pack per year history"?
HELP! I don't remember! :)

Pack year history is defined as an index of risk for cardiac and pulmonary problems. It is calculated by multiplying the number of years of smoking by the number of packs smoked each day.
Example: 2 packs per day for 20 years, would equal a 40 pack year history.
