Care Plan Format - Not Content Related

Hi,

I'm looking for a link, if one exists, that will help me with a premade format for careplans. Our school is requiring us to use a landscape presentation with columns for each area: Diagnosis, Goals, Interventions, Rationale & Evaluation.

I made one myself as a Word document, but I have problems with the columns getting all messed up when I have to make changes. If anyone is more computer savvy than I ( it wouldn't take much;) ) I would greatly appreciate some suggestions.

I have looked at the Ackley & the Evolve constructors as suggested by other posters in the two careplan threads & they both print in portrait format with no columns, so they won't work. Unfortunately. That would have just made my life too easy.

our school seems to do care plans differently than other schools. We have a patho page, Etiology page, disease concept page, medication page , a nanada page with goals and intervetions. It takes about 20 hours a week to prepare our care plans. If it is something you want send me your email and I will send you a copy. I can also send a completed care plan.

Did you check the stickys that have to do with careplans on this and the Nursing Student Assistance Forum? One of them has websites where you can view samples of care plans. A couple of those sites include a template. One of them is in a landscape format.
