Transferring a CNA License to another state

I have no clue how to transfer my current CNA license to another state. I am moving to Arizona from Wisconsin in Mid-July. I was told that Wisconsin requires the highest amount of class time to become a CNA so the license should transfer. I don't know if this is true, but either way, I dont know how to figure out how to get it transferred when I move down there. If anyone could help me that would be FANTASTIC!!!! I am moving to the Flagstaff area and I know there are a lot of nursing homes there, but have no clue how I can transfer the darn license. Please help!!!

Many states allow you to transfer just by filling out some paperwork and possibly paying a small fee. This process is called "reciprocity." You can get more details from the state's board of nursing. I was unable to find the exact form you need, but here's a link to a list of applications for Arizona:
