Doctors telephone orders???

Does anyone have any data on the proper way to take a telephone doctors order? I am a RN, but I need it for my job. A lot of new nurses and old are not doing this correctly. I just came back to this facility a month ago and I am trying to help improve the care of the residents. I use to have this info years ago, but things get lost over time. I know it myself, but is their a website with it listed or can someone like a DON or ADON help me out?

It's important I get something to show the nurses how to write orders taken over the phone.

Our healthcare organization requires:

09/15/05 0900 "Ativan 0.5 - 1.0 mg PRN anxiety"

R.B.V.O. or R.B.T.O. from Dr. John Doe to Jane Doe, R.N.

RBVO = Read back verbal order RBTO = Read back telephone order

Our orders include date,time,doctors orders(w/only approved abbreviations) doctor's name and the nurse taking the order. Verbal orders have been phased out. We only take verbal orders in emergent situations. All the doctor's are aware so they comply w/ writing ALL orders. Also we read back the order and document this beneath our names......
