Forms?

Hi,

I'm new to the site! Does anyone have any access to forms to use for faxing physician orders and for 3 month med reviews? I hate to re-create the stone if i don't have to....

Each agency usually has its own form for these purposes. They would have the logo or identifying info for that particular agency printed on it. Have seen some forms without logos from a previous agency but don't know if I have any lying around. Will look. You can always create your own, using word processing software, if you don't want to purchase commercial forms that may be available.
