Form for patient care ~ Clinicals

Ok...I'm tired of trying to flip through an almost 20 page care plan when providing patient care to try to make sure that I am covering all the information I need. Does anyone have a more simplified "all-inclusive" form that you use for patient care during clinicals? Thanks!

hey there!

i know daytonite has an awesome patient assessment form that is posted in all their messages and i used that one for the majority of my clinicals until my friend made the form that i attached. hope this helps ^^
