if a patient is hallucinating and says he hear some voices coming out of the wall, what is the right answer interms of

1 eastablishing nurse-client relationship

2 the most therapeutic answer

options are

1 egnore the pt.

2 i donot hear any voice

3 tell me more about the voices

4 its because you are hallucinating.

Q1: I would say mode of transmission because exposure is often due to being "social"

Q2: I would tell the patient I don't hear the voices in order to acknowledge their comment and to help them become oriented to reality.

Q1:I really do not understand the question, but I would eliminate pcp. "Social determinant" would be IV drug abusers, homosexuality and hemophiliacs I would chose "gender". :rolleyes:

Q2: I would say to tell more about the voices to determine if the patient was hearing "command" messages.

Q1... I would probably choose gender...

Q2... Honestly I would say its a combo of 2 or 3... Because I know you don't deny the voices they are hearing... So I would say... I don't hear the voices, but what are they saying... So having to choose one answer I would choose 3

Q1 I'd say mode of transmission-social isn't just limited to gender.

Q2-Tell me more about the voices. That's more therapeutic because you're assessing more about what the patient seems to be hearing. Heck, he could be hearing the TV in the next room, or hearing voices from space. THEN you could either wait until the patient asks if you hear the voices, or later tell them you don't hear them.

hello everyone,

i appreciate your enthusiasm to provide me insight about questions with enormous solutions to chosse most suitable one's, thankyou all.

i guess nobody will mind jogging up our mind :

q1 a 6 week old infant was brought to the clinic. the nurse noticed that husband was the one taking care of the infant, answering all questions and the wife was sitting in the chair, head downward, looking on the floor. what should the nurse assess?

1 psychosis

2 depression

3 stress

4 schizophrenia

can we chosse answer psychosis without the mention of the word postpartum, or simply chosse depression as the manifestations reveal. :confused:

I bet this question came from the incidents in which mothers murdered their children then claimed they were "depressed" or "hearing voices". In reality, the nurse cannot assess for a medical diagnosis, but can document obj/sub findings and encourage father of child to get mother to proper care. Being a pedi nurse I would probably alert Child Protective Services in order to protect child.

HI,

i didnot picked up the question from any senario and probably didnot got the back of this question.as i am taking classes for the prepration of rn exam and my teacher gave me this question.as from books i read about the postpartum psychosis (the case senario also favours me) but the teacher said no the symptoms are like the depression.My heart want to go with teachers' answer but my brain is not agrre------ing. :uhoh21: confussed thus want your help.
