Is it possible that the cause of a patient's problem could also manifest as a sign/symptom? I don't have an example in mind, but I recall seeing some example nursing diagnoses that seemed like they could be inverted. If there are instances of this, how often do they occur? Thank you.

Like a psychiatric condition with patients drink excessive water and cause hyponatreamia.

I am not sure what you mean...like CHF cause fluid excess?
OP I think ... I think ... means where the displayed symptoms are in actuality the cause/etiology themselves.
The last part regarding the nursing dx isn't coming across to me/making sense yet

We were taught to use the form: Problem related to..., as evidenced by ...
I am thinking there might be some examples where the R/T and AEB part seem reversed, or it seems like they could be reversed.
Sorry I don't have an example yet. Maybe it is a rookie mistake that would no longer happen after practicing.
